by using body temperature...erm,make it in simple way, heat transfer from hands when touching metal which lead to warming up inner vacuum flask. This is not all, i already dunno how to describe.. i can draw it for u all if u wan.. i have concept but dunno how to express..

use a bag in the bag setup. the inner bag is filled with hot water, the outer bag with warm water. this will protect against scalding if the inner bag were to burst, avoid the problem of the bag being too hot initially, and stay warm longer.

use a permanently sealed bag containing a soft substance with a large heat capacity which can be warmed up in a microwave oven.

Create a "flask" to keep a reserve hot water bottle hot. When your hot water bottle has cooled, you open the flask (probably by your bed) and use the new one
