Lack of proper Knowledge management, knowledge transfer. Investing in tools that help in better transfer of knowledge...sharing information...and a system to monitor each others' progress from time to time like using a project management website...like grouphubs..is a good idea.

in my opinion, the idea that a project is "too risky" is the greatest single threat to a project and nearly stamps out really good ideas. for great success to be permitted to happen, failure is certainly part of the picture. look at every single project -- failure is a component of all great sucesses. the greatest risk to projects other than fear is smallmindedness.

Uncertainty; at the very core risk occurs on account if incomplete knowledge which results in uncertainty

Unrealistic and over-optimistic assumptions. Lack for quality due-diligence exercise.

Poor/No planning

Cash/Funding running out before project is successfully completed.

Team members leaving the project before it is completed successfully

Team getting demotivated due to issues/hurdles in the project

Assumptions that are unsubstantiated or simply based on guesswork.
